Close Combat in Buildings

How does anybody handle close combat in buildings with several floors? It seems that close combat occurs when troops advance to within 4" of enemy, but distances appear to be measured horizontally only.

We treat enemy on another floor as not in close combat range, until one or the other side tries to change floor. Then the fight takes place, with the non-moving side usually defending light cover.

I think distance is measured horizontally or vertically.
So troops on one floor can engage troops on an adjacent floor
