This gets really exciting with tanks that historically could only fire a couple times a minute like the KV-2 or IS-2 with their separate charge & shell! But I appreciate the abstractions of CoC. Perhaps the panzer driver had some trouble getting it into gear and the Sherman had time to reload before he got it creeping forwards, etc. etc.
To me, using a CoC die like that is supposed to represent a tactical opportunity that is fleeting and uncommon (like the scenario presented above), but still plausible.

Yup, lot's of reasons the Sherman could have fired twice before the Pz IV including a dud round, a glitch with the traverse or site, a miscommunication between the crew such as the driver advancing before the loader and gunner were ready, misidentification of the location of the Sherman, and so on. Lots of weird and wonderful reasons because shit happens.
