Foundry Miniatures and Black Hussar both do civilian sets which have some useful options in 28mm.

The Perrys have an impressive surgeon scene in their napoleonic camp range https://www.perry-miniatures.com/produc ... onlookers/ and both they and Brigade Games have various civilians and clergy in their AWI, Napoleonic, and Carlist War ranges that could do as-is or with a head swap for period headgear.

The Foundry Cuthroats and Pirates range has some very good figures in more appropriate dress than Napoleonics - CUT014 "Horrible Henchmen" has an actual barber/surgeon, compete with medical bag over his shoulder, and another figure with a large book under one arm that could easily be a Bible.

Front Rank also do a range of 18th C civilians which includes a clergyman preaching and some well dressed gentlemen who could pass as a physic. The also have a range of Napoleonic civilians if you are not overly concerned with changing fashions.

As John says, Front Rank have some very nice civilians for the period. I also think Dixon minis have some too!
