Tbh, log bunkers or buildings prepped for defence and treated as bunkers are the only kind we've ever included in CoC games. Big concrete ones are (IMO) better represented by the kind of extra rules you get in the 1940 supplement.

No, hard cover is as good as you can get unless you're actually in a bunker. But they would still be counted as hard cover if targeted by HE.

Just activate the unit (team or squad) to move and fire at half effect. Use the movement distance to adjust the formation, then roll the shooting. This seems like a good way to do it. Agreed about 2). One CI per crew position.

For 1) I usually find there's always two that are closely than the third, even if it's just by a tiny smidge.

After two unbalanced opening rounds I wasn't unimpressed by von Luck. Maybe I haven't given von Luck campaign enough time yet, we will finish it and see how it pans out.
